House Bill 4224 Compliance Notice

In accordance with House Bill 4224 (89th Regular Session) and Texas Health & Safety Code §181.105, Larneka Lavalais, LPC PLLC provides the following information for clients and consumers regarding access to records, regulatory oversight, and filing complaints.

🗂️ How to Request Your Health Care Records
You have the right to request a copy of your mental health records.

To request your records:
Submit a written request via email to:
    - [email protected]

Include:
    - Your full name
    - Date of birth
    - Type of records requested
    - Preferred delivery method (secure email or other agreed method)
    - Requests are processed in accordance with Texas law and HIPAA guidelines.
    - You may be asked to complete a Release of Information form prior to records being released.
